Formerly of Angola and Dunkirk, NY entered into rest August 17, 2015 at age 75. Wife of the late John C. Polasik; dearest mother of Linda A. Guizzotti, Sherry (David) Battaglia and Janet (Chris Mariglia)VanLeeuwen; sister of Ruth (Frank) Perkowski, Sylvia “Sally” (Sam) Bennett and the late Shirley LoBello and Ralph Hammond; grandmother of Leah and Jillian Guizzotti, Nicole Miller , late Corey Kent,
Marijke and Annelise VanLeeuwen; great grandmother of Mason and Dante. Also survived by many nieces and nephews. Friends may call Wednesday from 3-8 PM in the Addison Funeral Home Inc. 262 N. Main St. Angola, NY where services will be held Thursday at 10 AM. Interment services will follow in St. Hyacinths Cemetery, Dunkirk, NY
